For your convenience and for my best preparation, I have provided a link to an online intake form for New Clients to fill out at their discretion prior to their first visit. I will also provide the option of filling out this paperwork on site 15 minutes prior to your appointment time if preferred, but it may save us both time to have it on file beforehand if possible. It is an extensive health intake form and I only expect clients to fill out what is relevant and applicable, but I make all fields available for a broad client base. Disclosing pertinent health conditions empowers responsible and effective therapy.
For all New Clients and Online Booking a personal account must be created following the "Book Now" link. This helps me keep track of your health history, your progress, your future visits and applying reward discounts for premiere customer accounts. It also allows me as your therapist to keep a virtual SOAP note (Subjective, Objective, Assessment, Plan) index folder for our sessions and elevates productivity.
I understand that unanticipated events happen occasionally in everyone’s life. In my desire to be effective and fair to all clients, the following policies are honored:
24 hour advance notice is required when cancelling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give 24 hours advance notice you will be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment.
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged for their “missed” appointment.
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.
